Statewide Fatal Crashes In MA, April 2023

Each month, we post about the fatal crashes in Massachusetts from the previous month, and share any trends that we see. For the full list of monthly posts, head here.

Last month, we took a look at crashes listed in the MassDOT Crash Portal in March; six were identified as people walking. In this post, we’ll look at crashes in MA in April 2023. The information in the chart below is compiled from news reports, and was checked against the MassDOT Crash Portal Dashboard “Fatal Crash Information.” Any Google Street View images included below use the address listed in the crash portal.

Of the 23 fatal crashes in Massachusetts in April in the MassDOT Crash portal, 5 were identified as people walking.
The average age of pedestrians hit & killed in April was 41.2.
3 of the fatal crashes took place at intersections.

According to a press release from the Essex District Attorney’s Office, 64-year old Silvana Seabra was struck by a van about 2:43 p.m. on April 3 at the intersection of Lawrence St and Erving [Avenue] in Lawrence. She passed away over a week later at Boston Medical Center. A man who also was struck suffered non-life-threatening injuries and was treated at Lawrence General Hospital.

The intersection is a street crossing for the Spicket River Greenway and is adjacent to the Hayden Schofield Playstead park; Google Street View from October 2019 shows a faded crosswalk.

According to the MassDOT Road Inventory, each road at this intersection is under local jurisdiction. Erving Avenue is a two-way street, with 1 travel lane in each direction. There is a sidewalk on both sides (note: road inventory says 1 side). The speed limit is unclear, as it is unlisted and not viewable nearby on street view. Lawrence Street is a two-way street, with 1 travel lane in each direction. There is a sidewalk and street parking on both sides of the street. The speed limit is unclear, as it is listed as 99 and not viewable nearby on street view.

WBZ/CBS Boston reports that 19-year old UMass Dartmouth freshman Frank Petillo, Jr. was struck and killed by another student on the campus’ Ring Road. As of this posting, a petition for pedestrian safety improvements on the campus had over 7,000 signatures.

From WBZ:

His death quickly drew attention to the perceived lack of safety on Ring Road, the school’s main thoroughfare and Chancellor Mark Fuller on Monday announced several steps to make it safer, either right away or by the end of the academic year.
They include portable speed bumps, flashing speed limit signs, additional patrol cars, elevated speed tables and crosswalks, and concrete barriers to protect some walkways.
UMass also said it plans to build a new pedestrian walkway to connect the school to a nearby apartment complex.
Students say these changes are long overdue. “The speed limit is supposed to be 25,” student Kyrillos Kedees said. “We really don’t want that to happen again.”

WWLP reports 29-year old Kyle Bezio was struck and killed after being hit by a car on Route 202 in the area of 150 Daniel Shays Highway. The 20-year old driver and his passenger stopped and tried to help Bezio before he was taken by ambulance to Athol Hospital.

According to the MassDOT Road Inventory, this road is under MassDOT jurisdiction. It is a two-way street, with 1 travel lane in each direction. There is shoulder on each side and no sidewalks. The speed limit is listed as 35mph in one direction and 45mph in the opposing direction.

41-year old Tammi Lyn Guy was struck and killed as she was crossing Jefferson Avenue at Wilson Street on the night of Easter Sunday.

Boston.com spoke to Salem City Councilor Patti Morsillo about the crash:

“My heart breaks for Tammi Guy’s family,” Salem City Councilor Patti Morsillo told Boston.com in an email. “This was a tragedy, and happened right in her own neighborhood.”
Morsillo said that after years of accidents on the residential and school-area street, the Salem Police Department is gathering crash data from the intersection. She also said that Salem will conduct a city meeting “within a week or two” after the collection process is complete. She says the meeting would include the acting mayor, the Director of Traffic and Parking, the Police Chief, the Lieutenant in charge of the Traffic Division, the City Engineer, the City Electrician, and herself.
“Instead of prioritizing efficient throughput of vehicles, we need to create safer roads for everyone: pedestrians, cyclists, and drivers. The goal for every road should be zero crashes,” she said.
“We will look at the effectiveness of the street lights, road markings, crosswalk visibility, and geometry of the road, to determine what needs to be done to make the area safer,” Morsillo added. “Salem has been working on intersection design upgrades in the area, and I am hoping to accelerate that work. The issue of road safety requires a team approach, which is how this work will move forward.”

It is a two-way street, with 1 travel lane in each direction; Google Street View shows that drivers create a second lane on Jefferson through the intersection where parking is prohibited, but not restricted. There is a sidewalk on both sides of the street and a crosswalks for 2 of the 3 legs of the intersection. The speed limit is 25mph.

53-year old Torrance Hodges was struck and killed by the 52-year old driver of a 2006 Chevrolet Express van who was driving southbound with the green light on Gallivan Boulevard at Dorchester Avenue.

According to the MassDOT Road Inventory, Gallivan Boulevard is under MassDOT jurisdiction. It is a two-way street, with 2 travel lanes in each direction. There is also a slip lane to Dorchester Avenue in each direction. There is a sidewalk and street parking on both sides of the street; street parking is restricted closer to the intersection. The speed limit is 30mph.

Statewide Fatal Crashes In MA, September 2022

Each month, we post about the fatal crashes in Massachusetts from the previous month, and share any trends that we see. For the full list of monthly posts, head here. Earlier this year, we released a year in review for 2021 to highlight common issues.

Last month, we took a look at crashes listed in the MassDOT Crash Portal in July; twelve were identified as people walking. In this post, we’ll look at crashes in MA in September 2022. The information in the chart below is compiled from news reports, and was checked against the MassDOT Crash Portal Dashboard “Fatal Crash Information.” Any Google Street View images included below use the address listed in the crash portal.

Of the 36 fatal crashes in Massachusetts in September in the MassDOT Crash portal, ten were identified as people walking.
The average age of pedestrians hit & killed in September was 68.8.
Three people in their 90s (91, 98, 99) were hit & killed in September.
29 year old Matthew Barker was struck and killed in a parking lot in Cambridge on September 1st at 1:22pm by a man in his 20s operating a box truck registered to a local restaurant. This crash is not included in the federal FARS dataset since it does not meet the NHTSA guideline for FARS (Fatality Analysis Reporting System). “To be included in FARS, a crash must involve a motor vehicle traveling on a traffic way customarily open to the public, and must result in the death of a vehicle occupant or a nonoccupant within 30 days of the crash.”

WCVB reports that 39-year old Thomas Ruffen died after Maximo Manzanett ran him over with a Ford Explorer SUV near the Jackson Square MBTA station. Police have charged the driver with murder. From the WCVB story:

Witnesses, including an Orange Line shuttle bus driver, told police Ruffen was on the ground outside the MBTA stop when Mazanett drove up in a brown Ford Explorer SUV. The shuttle bus driver said she got out of her bus and yelled to Mazanett to stop because someone was in the street.
“The driver said, ‘(Expletive) you, I work for the city.’ The driver then drove over the person, dragging the person under the SUV across Columbus Avenue,” according to a Boston police report.
Another witness said she also heard Mazanett yell an expletive before driving over the man.

MACDC shared a post in Thomas Ruffen’s memory, noting his work as an incredibly gifted organizer and community leader in the Mildred C. Hailey community.

According to the MassDOT Road Inventory, this intersection is under local jurisdiction. There is one travel lane in each direction on Ritchie Street leading into the signalized intersection with Columbus Ave and Centre Street. The speed limit is unclear, with one side of Columbus listed as 35mph and the other side listed as 30mph in the database.

The Boston Globe reports that a 32-year old man was seriously injured after being struck by the driver of a vehicle in Swansea. We could not find any additional news coverage of this incident; the person passed away from the crash. If you have any information, please let us know.

According to the MassDOT Road Inventory, this road is under MassDOT jurisdiction. There are two travel lanes in each direction and a painted median divider before the roadway adds another travel lane. There is a sidewalk on each side of the roadway. There are also on/off-ramps for RT-195 at this location with a crosswalk for people continuing along RT-6 / Grand Army of the Republic Highway. The speed limit is unclear, with fields showing 50mph and 40mph in the database.

WHDH reports that a 91-year old man was struck and killed by the 62-year old driver of a 2007 BMW SUV. The man had been in the crosswalk attempting to walk across Morton Street.

According to the MassDOT Road Inventory, Morton Street is under MassDOT jurisdiction. There are two travel lanes in each direction with one additional turn lane at the intersection. There is a crosswalk at each leg of the intersection. The speed limit is 30mph.

The Bedford Citizen reports that 99-year-old Josephine (Iovino) Caporizzo was struck and killed by the driver of a motor vehicle while walking near her home.

MassLive reports that a 48-year old man was struck and killed by the driver of a dump truck in Chicopee on Friday afternoon, September 16th.

Streetsblog MASS reported in late September that the “Act to Reduce Traffic Fatalities” was sent from the Governor back to the legislature with recommendations for changes before it would be signed. This bill as written includes additional regulations for some trucks to include convex and crossover mirrors, backup cameras, and lateral protective devices.

The Gloucester Times reported that Virginia “Ginny” Hale, age 98 1/2, was struck and killed by the driver of a box truck as she was crossing the street by Cumberland Farms on Railroad Avenue in Rockport. From the article:

Virginia “Ginny” Hale, 98 1/2 — she insisted on the half, said her daughter-in-law — was a daily walker, going out in the mornings and late afternoon and covering miles.
“She would greet people along way and visit the local shops,” said Betsy Hale, her daughter-in-law, adding she never meet a stranger. “She was a positive, loving fashionista. She always dressed up, did her makeup and did her hair to go out.”
A life resident of Rockport, Ginny and her sister, daughters of Swedish immigrants, were brought up by family after her mother died. Her daughter-in-law said her love of beautiful clothes probably started with the outfits her aunts saw that the girls wore. Photos from the 1930s show Ginny wearing suits and gloves when she hopped the train to Boston — she never drove — where she was an office worker.
“She always dressed to the nines,” Betsy Hale said. Many mentioned “Ginny sightings,” describing her outfits, and Betsy Hale said she thrived on the compliments. and she devoured fashion magazines.

Wavelengths/97.9WHAV reports that a 79-year old woman was struck and killed by the driver of a 2009 Volkswagen Jetta on Winter Street near the intersection of Locust Street.

The Boston Globe reports that 56-year old Dawn Squires was struck and killed on Route 28 in West Yarmouth by a driver of a truck from New York.

In 2016, Callie Crossley penned a commentary for WGBH titled, “Brought to Light: The Case for Street Lights on Cape Cod. It’s Life and Death“:

I know there are residents who are reluctant to mar the horizon with a string of street lights. Easier to see the moonlight on Cape Cod Bay that Patti Page croons about so sweetly. But, if my unobscured view must be sacrificed to save someone’s life, then so be it.

According to the MassDOT Road Inventory, this road is under MassDOT jurisdiction. There is one travel lane in each direction and a sidewalk on each side of the street. (However, the most recent Google Street View image is from 2019, and shows a rolled curb sidewalk on one side of the street; the crash photo from CapeCod.com also does not show a sidewalk on one side of the street.) The speed limit is 35mph.

Each month in 2021, we plan to post about the fatal crashes in Massachusetts from the previous month, and share any trends that we see. For the full list of monthly posts, head here.

Last month, we took a look at the five fatal crashes listed in the MassDOT Crash Portal in September. In this post, we’ll look at crashes in MA in October 2021. The information in the chart below is compiled from news reports, and was checked against the MassDOT Crash Portal Dashboard “Fatal Information by Year.” The Google Street View images included below use the address listed in the crash portal.

Of the 28 fatal crashes in Massachusetts in October in the MassDOT Crash portal, seven were identified as people walking.
The crash portal does not include names. The name of two of the people walking who died have not been made public yet.
The average age of pedestrians hit & killed in October was 65.
At least one of the crashes were hit & runs (as referenced in news articles).
The name of the person driving was only identified in one of the crashes in news articles that we found.
We could not find any news reports for one of the fatal crashes.

According to the MassDOT Road Inventory, this street is under MassDOT jurisdiction. This stretch of Huttleston Avenue (State Route 6) is two-way, with 2 travel lanes in each direction. There is a sidewalk on each side of the road. The speed limit is unclear in the MassDOT Road Inventory, with 25mph and 35mph both listed.

The Eagle-Tribune reports that a driver struck and killed 73-year-old Antoquia M. Ruiz Villlona De Anziani in the area of Essex Street & Winter Street in Lawrence. The driver was not identified.

According to the MassDOT Road Inventory, both of these streets are under local jurisdiction. Each road is two-way, with 1 travel lanes in each direction. It is a 2-way stop, with stop signs for the Winter/Medford Street approach (Winter Street becomes Medford St as it crosses Essex). There is a sidewalk on each side of each road. There is no painted crosswalk over Essex Street. The speed limit is unclear in the MassDOT Road Inventory, and not obvious on Google Street View.

The Enterprise News reports that Joao Fernandes, age 54, died after the driver of a Mel’s ice cream truck struck him at the intersection of North Main Street and Huntington Street. The driver was not identified.

According to the MassDOT Road Inventory, North Main Street and Huntington Street are both under local jurisdiction. Each road is two-way, with 1 travel lane in each direction. There is a stop sign on Huntington Street as it meets North Main Street. There is a sidewalk on each side of each road. There is a crosswalk across Huntington Street, but no painted crosswalk over North Main Street. The speed limit is 30mph in the MassDOT Road Inventory.

Lisa Vadeboncoeur was killed and two other people were injured after 46-year-old Kevin McCaffrey struck them on Theodore Glynn Way in Boston. WHDH reported that he has been charged with motor vehicle homicide by operating under the influence of drugs, negligent operation of a motor vehicle, and two counts of assault and battery with a dangerous weapon. Their coverage included a statement from Suffolk District Attorney Rachael Rollins:

“Ms. Vadeboncoeur and the two other individuals injured in this crash were standing well off the side of the road when they were struck,” Suffolk District Attorney Rachael Rollins said in a statement. “My office has been in contact with each of the victims and the loved ones of Ms. Vadeboncoeur. We will continue to be available to provide them and the two surviving victims with the support and resources they may need and that they deserve.”

According to the MassDOT Road Inventory, Theodore Glynn Way is under local jurisdiction. It is a one-way road that is 64-feet wide; there is parking on the left side. Street View from November 2020 does not show any lane markings. There is a sidewalk on one side of the road and a shoulder on the other side. The speed limit is not noted in the MassDOT Road Inventory.

A 79-year-old man was struck and killed by a driver on North Quincy Street in Brockton. Neither the driver nor the pedestrian were identified.

The Enterprise News observed that this was the second fatal crash in a week in Brockton.

According to the MassDOT Road Inventory, North Quincy Street is under local jurisdiction. The road is two-way, with 1 travel lane in each direction. There is a sidewalk on each side of the road, but there is a large curb cut on one side shared by the Brookfield Plaza and a used car dealer. (The Enterprise News story includes photos of police investigators examining this side of the road/driveways.) The speed limit is 30mph in the MassDOT Road Inventory.

A bus driver hit and killed Jennifer Formichelli and then left the scene at the corner of Walk Hill Street and Blue Hill Avenue. CBS Boston reported that Formichelli was walking her two dogs at the time. Neighbors said one stayed by her side, and the other ran home, alerting neighbors. The Boston Globe reported that bus and driver was not affiliated with Boston Public School system, but the police have located the driver and school bus, and homicide detectives are investigating. The driver was not identified.

CBS Boston also interviewed Fillmore Parris, who was engaged to Jennifer:

He said Formichelli was an English teacher, loved her pets and her family. “She was just an all-around good person.”
“I’m sad but I’m just getting increasingly angry,” Parris said. “I don’t care if you’re driving a tank, you know if you’ve hit somebody, especially somebody that is as visible as her, running with two dogs.”

It is a two-way road that is 40-feet wide; there is parking and sidewalks on each side, and there is a crosswalk across Walk Hill Street. The speed limit is 30mph in the MassDOT Road Inventory. There is a traffic signal at the intersection of Blue Hill Avenue and Walk Hill Street.

66-year old Fabio D’Assis was hit and killed by the driver of a Toyota Highlander SUV. The Worcester Telegram & Gazette cited a police spokesperson that “the initial investigation indicates the driver approached the Auburn/Worcester line and saw the pedestrian, but was unable to stop in time.” The driver was not identified, but is described as a 42-year-old Sudbury woman.

The T&G story locates the crash near 557 Southwest Cutoff, while the MassDOT Crash Portal lists 528 Southwest Cutoff. 557 Southwest Cutoff is closest to the Auburn/Worcester line referenced in the article, so the Google Streetview included below reflects that section of road. There is a grocery store plaza on one side, with housing on the other side of the road.

According to the MassDOT Road Inventory, Southwest Cutoff is under MassDOT jurisdiction. It is a two-way road that is 48-feet wide, with 2-lanes in each direction; there are sidewalks on each side. The speed limit is 35mph in the MassDOT Road Inventory.

Statewide Fatal Crashes in MA, February 2021

Each month in 2021, we plan to post about the fatal crashes in Massachusetts from the previous month, and share any trends that we see. Last month, we took a look at the 5 fatal crashes listed in the MassDOT Crash Portal in January. In this post, we’ll look at crashes in MA in February 2021. The information in the chart below is compiled from news reports, and was checked against the MassDOT Crash Portal Dashboard “Fatal Information by Year.” The Google Street View images included below use the address listed in the crash portal.

Of the 19 fatal crashes in Massachusetts in February in the MassDOT Crash portal, 4 were people walking.
3 of those 4 crashes were on limited access highways. In 2 of those crashes, tractor trailer drivers struck and killed a driver who exited a vehicle after it had broken down.
The crash portal does not include names. The names of 2 of the people walking who died have not been made public yet.
The name of the person driving was not identified in any of the 4 crashes in news articles.
2 people lost their lives at a construction site in Boston in February when a co-worker hit them with a vehicle, pushing them into an open trench. That incident is not listed in the MassDOT Crash Portal. The Boston Globe wrote at least two follow up stories about 33-year-old Jose Carlos Figueroa-Gutierrez and 27-year-old Jordan Romero: “One day later, makeshift memorial for two construction workers only sign of tragedy in Boston,” “A sister mourns for brother killed at downtown Boston construction site.”

A 33 year old Hartford man was struck and killed on on I-90 (the MassPike) by the driver of a tractor trailer. WHDH reported that he was hit after his Toyota Camry broke down and he got out of the car to examine the problem.

A 70 year old woman was hit and killed on Winthrop Ave (Rt 114) on the North Andover / Lawrence line. The Boston Globe reported that the scene of the crash on Route 114 was near the North Andover Mall. Rt 114 is 4 lanes (2 travel lanes each direction) in front of the North Andover Mall and 3 lanes after the driveway intersection. There is only a sidewalk on one side of the street. According to the MassDOT Road Inventory, Winthrop Ave/Rt 114 is under MassDOT jurisdiction.

According to Google Streetview, the sidewalk is relatively new and under construction as of November 2019. There is only a sidewalk on one side of the street, but it did not exist prior to November 2019. New curb ramps are in place but only go across the driveway entrance, not Rt 114. The nearest crosswalks across Rt 114 are 0.9 miles away from each other, with the shopping center roughly in between. A housing development is also on the other side of the street from the shopping center.

Mustapha Faddi, a 57 year old Marlborough man, was struck and killed on I-90 (the MassPike) by the driver of a tractor trailer. Mustapha’s Nissan Murano broke down and he got out of the car to examine the problem. The Patch shared that this was the second fatal crash on the MassPike in February involving a disable vehicle & tractor trailer.

Madeleine Fox, a 22 year old woman, was hit and killed after she got out of her friend’s car on I-93. NBC Boston reports that investigators are working to identify a description of the vehicle or vehicles that might have hit her.
